Claim Form

If you at any time between 2014 and July 9, 2025, made a purchase of an item from a “Subject Vending Machine” in the United States using a credit, debit, or prepaid card, then you may be eligible for settlement payment.

To receive a settlement payment, you must complete and either submit online, or postmark and mail for return, a Claim Form on or before November 14, 2025. You will not need to submit proof of purchases with the Claim Form. However, you may subsequently be asked to provide additional information. If you are a Class member and return a completed Claim Form that is timely and valid, and if the Court grants final approval of the settlement, you will receive money from the settlement and be bound by all orders and judgments of the Court.
